Congratulations to all of the teams that won their respective conference tournaments this past weekend and for claiming a spot in the potseason. For a full list of all of the conference tournament champions, who serve as the 31 automatic bids for this year's postseason, as well as the 16 Regional sites that were announced by the NCAA on Sunday evening, visit Mike Rooney's final Field of 64 projection.

This will be the last Top 25 ranking before the final, final one, which will be posted after the College World Series. For now, moving Texas A&M to the top spot was a pretty easy choice. They were neck-and-neck with Mississippi State a week ago and beat Florida on Sunday to win the SEC Tournament. As a result, Florida also moves up to the second spot.

While Miami holds onto their No. 3 ranking despite not winning the ACC tournament, Clemson moves up from 20th to eighth given their incredible week of play. They have been especially tough to beat lately after hitting somewhat of a midseason slump, but now are firing on all cylinders as Regional play opens later this week.

We're also going to hand out some hardware for the top player and pitcher of the conference tournament week, thanks in part to our partners at Rawlings, and will continue to do so through the Regional and Super Regional rounds.

Perfect Game/Rawlings Player of the Week: Luken Baker, Texas Christian
Baker was already in the discussion for the Player of the Week Award prior to his Sunday heroics, hitting a solo home run in extra innings that proved to be the winning run as TCU beat West Virginia to claim the Big 12 Championship. Baker, who participated in the 2014 Perfect Game All-American Classic, was nearly unstoppable, going 15-for-23 (.652) in five games with a double, four home runs, 10 runs scored and 11 RBI.

Perfect Game/Rawlings Pitcher of the Week: Anthony Kay, UConn
Kay, one of the top prospects available for this year's draft, had two huge wins for UConn, including Sunday's performance that guides the Huskies to the American Athletic Conference Championship. Kay went six innings on Sunday, allowing two runs on six hits and two walks while striking out five as UConn beat Houston to claim the title. Chris King was on hand for Kay's first appearance in the tournament, a complete game victory over Memphis on Wednesday (read those notes and watch video of Kay from that game here). Overall Kay was 2-0 with 15 hits allowed in 15 innings, striking out 11 and allowing just four walks with a cumulative ERA of 2.40.

Don't forget to tune in to Perfect Game College Baseball this evening from 5-8 p.m. ET on SiriusXM College Sports Nation, channel 84, with hosts Daron Sutton and Mike Rooney, as they analyze the postseason and upcoming Regionals after the field is announced at noon eastern. We will provide a list of the field once it has been released.

Also considered:
 Dallas Baptist, Rice, Saint Mary's.
